Emergency Medical Services
The Eye Specialty Group is the only eye care facility in the area that offers ophthalmic emergency service care. Our team of highly trained specialists is available 24 hours a day, 7 days a week to examine, treat, and triage patients. New Patient Preregistration Forms
To streamline your check-in time for your initial appointment, you may complete the forms, print, and bring with you for your first visit.
The forms have entry fields that can be filled in by placing the blinking cursor in the entry field areas. Press the “tab” key to go to the next entry field.
Please make sure the forms print properly before closing. Additional information will NOT be saved once closed.
Our billing office is available to answer your questions regarding insurance, billing, and payment information.
Please bring your insurance cards when you come to our office for your first appointment. The Eye Specialty Group physicians are Medicare participating providers.
Many medical insurance companies as well as HMOs require preauthorization for medical services. Please consult with your insurance company or primary care physician to obtain a preauthorization for services, if required, prior to the date of your appointment.
We accept cash, checks, Visa, Discover, MasterCard, and American Express for payment of services. Co-payments and payment of noncovered services are required at the time of your visit.
